Facebook App/page Code Clone 2

Hi,
I’m not very familiar with how Facebook’s FBML code works, so I need someone to copy the functionality on this page for me. I just started researching this today, but it looks as if I need a facebook app to make this custom page work.
Here is the custom page: (the tab titled “Apple iPad Giveaway”)

I need code written that replicates the actions of the Step 1, Step 2, Step 3 buttons on this page.
You don’t have to do any graphic design work. You just have to write the code to make the buttons work.
I need this done quickly, and need instructions on how to set it up on my facebook account.
