I need someone who is experienced with creating forms for submission of data on a website.

I need basically 2 forms created. One form is fairly long but not overly complex and the other form is short and simple.

These forms are for a loan application website.

In order to get an idea of what the form will require simply search a few mortgage or loan websites and see what the forms look like and how they behave.

I will provide you with a few example forms, as well as all the fields neccessary and any validation/rules for the form.

The forms data should be sent to my email addy, with the data also stored in a database if its not too much extra work just in case I have problems with my email for some reason.

Please include revisions to the layout/look/validation of the form with your bid as Id like to have it just right.

The 2nd form will just be a condensed version of the full, long form. So you are really only creating 1 form.

Form should also be coded well, and not take a long time to load.

Also, it would be good if I can add a SSL certificate. Not sure if you would do that part or if you can help implement that as well. I do have a SSL from another website Im not using anymore, so I can use that one hopefully. I would only need the SSL on the main, long form to ensure clients data is secure.

We are looking for a simple flash video overlay player like the one found ones found on VideoSpokesModel sites. We will produce the videos and have a streaming & progressive flash hosting account available.

We film actors on a greenscreen. We then take that footage and key out the green and create an alpha channel. We then convert the video into .flv w/the alpha channel.

I am developing a project to create a bilingual website (English/Dutch) (similar to skiportaal) to advertise (Summer and Winter) special offers and last minute deals imported automatically from selected tour-operators database per country.
The website would incorporate a script to enable search and download of the special offers and last minute deals from the selected tour-operators database and be able to customise the imported data so that all Ads would have the same lay-out and structure although they are imported from different tour-operators.
This concept is similar to http://www.skiportaal.nl/ but rather than a link to the tour-operator site (like this affiliate program does) my website would have a 0900 paid call number through which holidaymakers could contact touroperators directly for further information and/or booking.

Actually, not even rearranging. Just making the header look the same in IE and all other browsers out there.

Same goes for the contact us page which has the text field moved to the right in IE.

No designing required. Just moving the stuff left or right so they look the same in all browsers.

I am currently pulling together functional aspects of existing sites which I would like cloned / mirrored and then some general layout / design ideas incorporated.

A straight forward owner based maintenance function with no need for web support.
A named related Logo. Details to be provided.
‘Slots’ for up to 28 auctions on the main page which each having count down timers, images, product details and also a quota percentage facility, that increases as the auction recieves more bids. (More info on request.) These slots also require a sms / text number entry for bidding via mobile phones).
A scrolling text bar function.
A membership / login / function etc.
Credit Card and Paypal payment facility.
A ‘Bidding credits’ cash conversion function which changes peoples cash payments to credits for bidding.

I need my web form validation scripts modified in order to perform the desired actions. A minor web design change is also required. The first form ( http://azhearing.com/catalog-request.php ) needs to trigger a php file ( http://azhearing.com/mail.php ) to email me ALL the form results while posting them at the same time to http://www.azhearing.com/cgi-bin/PDG_Commerce/catalog.csv (Script needs to create this file if one does not exist.) Once form is submitted, the user will read on the screen, “Thank you, <firstname>. Your catalog will me mailed out soon. Indeed, we are creating a site of clairvoyance and components provided do not meet our demand. The site will contain a dozen soothsayers to give visitors subsequently named “Customer”.

The client can credit his account (I think we’ll use Paypal) to consult one or more soothsayers. Course credits not used it will refer the same or another soothsayers at its next visit. It can of course see many soothsayers on the same day.
