Contact Us Page – With Backend
We need a simple contact us page.
We could do this in house, but we’ve never used scriptlance before. Since we will have additional projects going forward, this project is a way for us to try scriptlance.
We require the styling and css the same as the home page with the same footer and header. It should be clean in style.
It should have fields:
Type of contact: (menu: Inquiry, Comment, Support)
Name:
Email address:
Phone:
Message:
It should have captcha and a question validation.
The form should capture ip address.
The admin page should be username & password protected.
It should provide a listing of messages that can be opened to be viewed.
jquarry is being used on the site.
