We need to build a dynamic web site for a restaurant. We need design and functionality for this project.
The main page will have the logo of the restaurant on the top left corner and a flash animation on top, the menu will be on the left side below the logo.
There will be a section for news, special events and special offers on the main page.
The menu items will be:
Home, Food Menu, Wines, Map and Directions, Contact Us
The Food Menu page will show all the menu options and will allow users to search food items based on the name, type of food, price, ingredients or wine.
Ability to create the menu on the fly as a pdf.

The Map and Directions page will show a Google map.

We need a simple 3 page website with some custom scripts. Here are the requirements:

Login on home page for users. Logins to be created through an administrative back-end.

Once logged in user fills out a short form indicating their Age, phone number etc. Information gets stored in MySQL database.

User verifies the info on next page, after verifying they click “Finished”

At this point the user has the option to print a card with the information on it through their browser, so the data they put in the previous step would have to be dynamically inserted on a printable card.

The data in the database also gets compiled into a text file that is upload to an SFTP at set interval via CRONJOB.

User logins are created from administrative back-end User info that has been entered can also be viewed from admin back end. (Exported to CSV as well)

The site will be secure https with a self-signed certificate.
