Authorize.net Integration

I am looking for someone to integrate my site with authorize.net, the php codes (api) will be provided, I just need someone to build a nice form with mysql database backend for customer information storage.
The header and footer are already provided for the forms. This should be fairly easy to do since the api’s will already be provided. I can show you a few websites that I would want it to look like. Please contact for more information.
