Apply New Template To Php-site

Hello, Looking to apply a new template to a php website. The website is a relatively simple site. mostly text pages, and 2 forms. (all of which is handled by a backend db. which I can connect later.
I require a php/css/xhtml programmer to change the current templates to the new design. the PHP file structure is attached.
You will need experience with php templates, xhtml and css.
