Joomla Quote Form And Uploads

Products already have partial form with product info made. Need to add to that form.

Form data goes to email and also on joomla site where it can be checked.
All products need form.
Create a new page for file upload where User Name, Project Name, and Order number are added to file upload with preview of file/image.
