Youtube Video Download Script

I need a script that will allow users to download youtube videos by inputting the video link, and I would like it to support mp4 or avi or both, as well as an option to rip sound only for mp3. I also need it to support css themes.
Some demo sites I found:
http://www.listentoyoutube.com/ – This site takes video and extracts mp3 only
http://keepvid.com/ – This site downloads videos
I have seen sites that download and convert, and have the option to download mp3 only. I want all these features.
I would also like it if there were different themes, or support for css themes.
