Add Login Box To Html Page 2

I need someone to add a Member Login box to my home page. I have a custom HTML landing page for my website on my main index, and the login database script I am using is phpfox.
**Landing page is on main domain – mysite.com
**Member login script is in a sub-directory – mysite.com/phpfox
I have attached an image of where I want the login box. If you have any question, feel free to ask.
