Feedback Widget

I’m looking to have someone create a script for me similar to the feedback widget at http://www.getsatisfaction.com/widgets
The functionality would be the same, except without the community features.
This would also include a system for the administrator to create the widgets for multiple sites, and then to view the responses in a dashboard. This part of the project would be written in PHP/MySQL.
