Windows Installer Program

We are looking for a windows installer program to install a program written in C.
Here are the details:
*- User must accept EULA to install
*- Ask for Client # (**see note bellow)
*- Create a folder on the computer
*- Install exe program in new folder
*- Create a service that will automatically start with windows (for all users)
*- Installer must work for Windows XP, Vista and 7
Note: Client # will be used to identify the software and used when program runs for the first time
No need to give your full resume, please show us any screen shots you have of installer programs you have created or will be using.
