Mysql Php Form To Database

I have created a form (which I am happy with) and it will write to MySQL database by a . I need someone to take the form and SQL and do the following:
1) after form writes to database, email the user and also create on-screen printable version.
2) the saved data needs to be accessed by the user via a secure ID/password login page and then the user can update and save the revised data, with an email copy and print copy
The work can be done on my server or on yours (I’ll supply the SQL and the PHP form).
