Web Designer And Programmer

Hey,
I’m looking for a talented website designer and programmer who can customize HTML code and who can blend forms on websites. An example would be this site:
http://6figureincomechallenge.com/Project2/1/landing.html
The task would be to “blend” the page with this form so that the background was brown and add text above the form to customize the page.
Looking for a long term web programmer/developer/designer who can customize many pages per day. To start off the relationship I would propose testing 2 pages.
