Need Mobile App Daily Journal

We want to design a Mobile Application for users to be able to either log onto the Internet via their PC or mobile phone where they can add entries into a daily log of activities. The application should allow them to retrieve a report in excel or otherwise as to their activities for the daily, month or year.
