1. From our quick links drop down menu, we want that application form to link to the form you created. You can remove the “application information ” link under admissions, we just want it in the quick
links
Once on form…
2. once at your form, the user has two options, 1. to download this form, http://woodlandsprep.org/UserFiles/CompleteApplication09-10.pdf
and fill it out later and fax it in later or 2. proceed to the online web form that was create in php
3.If they choose 2, proceed to the php form and fill out the form, sections not needed are, “Parent Request and Teacher Recommendation”, those two sections need to be put in a pdf format and at the end of the form, they need to be available for download. See those two sections the information as to be filled out by the teacher, so there’s no way that can be done at that time.
