Php/javascript Expert Needed

I’m looking for an expert in PHP/Javascript to code an my opt-in form that collects e-mail address to do the following:
1-send men to thankyoupage1, send women to thankyoupage2
2-sign men to list1, sign women to list2
Perhaps you could pass the CGI variables to the thank you page. I am with Aweber.
