Survey Website (vb/asp.net)

Hello,
I need a survey Website, written in VB/ASP.Net and SQL Server.
We are developers, but don’t have time to do this ourselves.
User : Login and depending on details in database, present relevant questions.
Admin : Create surveys, create users, view results, report results (with charts)
All code to be well documented & commented, web design to be clean with css.
Please only bid if you have the VB/ASP.Net code ready to go (or something very similar).
I do not want this developed from scratch.
Please send me either a link to your existing similar work or a screenshot.
MUST BE VB/ASP.Net & SQL Server.
