Excel To Access

I need the attached Excel spreadsheet converted to an access database.
The source data will be imported via a text file (which I will provide). There needs to be a utility to browse for the text file and import, and then the user will input as per the spreadsheet to generate a salary increases As is > To Be and then print out the result.
