Twitter Quiz Application

I am looking for a simple quiz script for our website.
I want it to be the same as this tattoo quiz and for it to be connected with twitter (answer the 5 questions and you’ll see it’s uses twitter login). It then gets posted in Twitter that you took the quiz with a link to take the quiz.

Would like to be able to change the questions easily.
Please give explanation and experience in PMB.
