Contact Us Page With Css Form

Create a contact us page with PHP Script to email the results to an email address. The page template will be provided and the 7 fields and other details will be provided.
The form should look and act like the one at: http://tetlaw.id.au/upload/pages/really-easy-field-validation/
using technique at http://tetlaw.id.au/view/javascript/really-easy-field-validation or http://livevalidation.com/ or similar method.
The code should resolve well in all common browsers.
Attached is the page template.
