Wap Perl Subscription Form

I need a WAP subscription form that works with my PERL email program.

I think it may be better for the subscription form to be broken up into separate pages.

For example first they select if they want to subscribe or unsubscribe, then they select which list (if more than one list is available) then they select if they want to unsubscribe their cell phone email address or their internet email address.

With this project I have included attached code as to how it would look all on one page.

I would like someone that is familiar with how this might work on the cell to see if there may be a simpler way than what I am asking. Also because of the cost I would want whatever is more inexpensive done first as long as it works and is not difficult for a cell phone user to manage to subscribe or unsubscribe.

I am including the form in it’s entirety.

The form now has the carriers coded but would prefer if this was pulled from the configuration file config.pm where it is stored. In the attached file I have the code I am mentioning and more regarding how this might be called.

When the form completes processing the subscription there are other files that may be called such as one saying

thank you for your subscription. Another possibly saying you are already subscribed to this list or yet another explaining they have been unsubscribed. I will need that to also be included in this project.
