Create Web Forms
I have 12 pdf forms, some with lots of data to enter, others with just a name, date and signature line. These pdf forms are just static pdf’s with no entry fields allowed at the moment. I have these forms in .doc format as well so they can be converted to fillable pdf forms. I have a joomla website on which i want to put these forms on.
The project entails converting the pdf forms (and the entry fields) into a fillable pdf form which in turn will capture the data and send the forms via email to a specified email address when the user hits submit (storing data in a MySQL DB as well is a possibility). I would like it so that the user has to fill out all 12 forms completely if not they cannot submit the forms (this can be optional if not possible). The order of the forms is specific. The purpose is to obviously have the user fill out a form which looks like a real form (not a web form) and be able to apply online.
I can supply all forms for review and analysis if needed. This project is ready to start immediately. I need this project within 2 weeks.
