Javascript Auto Populate

On this page http://www.evansgr.com/dev/assign-claim.php I want to
automatically populate fields if certain conditions are met.
If “Adjuster” is selected for the first field then populate the
following fields in the Insurance Information section that appears lower on the form:
Adjusting Company –> populated from Your Company
Adjuster Name –> populated from Your Name
Address –> populated from Your Address
City, State, Zip: — > populated from Your City, Your State, Your Zip
Phone –> populated from Your Phone
Fax –> populated from Your Fax
E-Mail –> populated from Your Email
