Ajax Form Calculation Needed 2

Hi we need to have the same type of ajax form calculation method done for our site as this site.
http://www.w a v e s t r e a m i n g .com/servers/shoutcast/ and click the customise server link,
I will need to be able to change values and also be able to copy the form to another few pages with different values, so will need instructions on how to add or edit fields/values
