Need Simple Mysql Db Created

We need simple MYSQL db created from the following:
1. We have purchased a data list from a data company (if needed DUMP FILE for MYSQL is also available). Please see attached technical file from the data company.
2. Page scarpping of couple of websites will also need to be done to add to the MYSQL db created.
We will need this db to be fully searchable from our website. You must also install the MYSQL db on our server when completed.
PLEASE NOTE: Please do not ask for advance payments as our company will not authorize any payment without any work done. We will fund escrow and will release payments as the milestones are completed.
