Build Good Looking Form

Need someone to build a good looking enquiry form and integrate using Form Post to my lead broker.
I have a full set of instructions from the broker for you to follow and I have attached the foll documentation..
Here is the first line of the instructions:
Each lead channel that uses the Form Post delivery method has a unique POST URL that the Form uses to submit data to LeadPoint using the HTTP Post method. The Posting URL includes the Lead Channel ID for the channel you are creating. You will use this POST URL for your Lead Channel both when it is testing mode and after activation.
