Search Box Integration

We want to have a search box integrated in a website. This search box have the scroll options: first: country and then city. After pressing the search button the visitor will be directed to a predefined url.
The design of the search box should be translated in html/css and the code should work in html and php. All countries, cities and corresponding links or managed in a mysql database.
