Online Registration Form

I would like to know how much it would cost to set up an online registration form where students can sign up for a group study seminar or to see a tutor and fill out a brief questionnaire if it is their first time. The online registration form should be simple and easy to use. Students should be able to sign up online easily and fill out a brief questionnaire. The students should be able to sign up to attend a group study seminar or to see a tutor. I would like to know the cost difference between setting up a new website or if I already had a website and I just wanted to add this feature to my existing website.
I would like to know the cost to implement an online registration form and how much it would cost to maintain this each year.
