Need Php Form Fixed 2

I have a form that you fill out then it brings you to paypal where the customer pays, then once they pay it submits a confirmation email to me and one to them. I had a programmer put the form into a new layout and he broke the form. I just need a programmer to correct the form to do what it was originally doing. I have attached the original form and the new form so you can see and correct it. I need this done within 1 hour of being selected.
