Custom Form WordPress Plugin

I need a custom form similar to CForms 11 (http://www.deliciousdays.com/cforms-plugin) or Formbuilder (http://wordpress.org/extend/plugins/formbuilder/) but with a postcode (zipcode) field in the form that will read from special admin area where I will need to be able to add several mail recipients and associate them to selected postcodes which I can manually add via the admin.
I need to be able to change them if I need to.
This plugin should be like other wordpress plugins and have its admin situated inside the wp-admin.
