Simple Website Modification

Hi,
Need a programmer to help me with a few things..
1 – Install my logo on my PHP website (21 php files in total)
2 – add my forget password link on my other website: 3 pages
3 – add a bonus section to one of my site (3 exact html pages but same content). *I have the text and picture
4- Remove a text box from one of my site and replace with another one (need to be done on 3 html file)
4- I need a very nice contact us form with captcha
I need this done ASAP
