Php/ajax Uploader

I need an ajax file upload script created with progress bar showing the status of the upload. Also I need the upload script to be written in php.
Also the php script after the upload is done I need the script to send the client a confirmation email letting them know that the upload was successful, Also the script needs to send an email to the site owner letting them know that there is a new upload with a link to the files in the email.
I am attaching the way the upload section should look. I need the upload section to be styled exactly the way it look.
