Html Generator Modification

Hello,
I have html I would like turned into a widget that people can embed on other sites. Right now I’m using an html generator.
I have the html written in the textarea as a default with indications on where they should copy/paste their picture url and where they should paste in their 2 url links.. They hit generate code and woola!..
Well Instead of the user seeing the whole code in textarea, I simply want them to put their picture and link urls in different input fields, hit submit and woola!… shows preview and spits out embed code.
Also have another simple project related to this one for the right programmer.
