Database Errors During Install

Hello,
We are trying to install a website that has an installer script. You create the db (mySQL) and then fill in the relevent fields. I am getting an unknown error.
I need someone WHO IS A PRO AT MYSQL & PHP to take a quick look and tell me what is wrong.
Easy money if you know what you are doing. $5 and great feedback will be awarded.
