The pdf is divided into several worksheets. The explanatory text and questions in each worksheet should be all on one form. so you will be creating several different HTML forms, one for each worksheet. All text and questions in the pdf should end up on a form. I’ll divide the HTML forms later into smaller chunks if I have to.

Use the HTML label tag with the ‘for’ attribute and the proper type of input elements based upon the question. Don’t add any <p> or <div> tags. I’ll style the entire form using css later. Name each input element using camelCase with an explanatory name based on the question being asked. Don’t abbreviate the input names too much – only where it makes perfect sense. Use both a ‘name’ and a matching ‘id’ attribute for each input tag.

Please use the jquery English validation library for form validation as appropriate. All questions are required.

Write a single PHP script (or better yet a PHP class – specify in your bid which you will do) that will take the submission of any form. The script should check for the existence of a field that has the same name as the input element in a MySQL database table. If the field exists, update the field with the contents of the POST. If the field does not exist, add it to the table with the POSTed value.

The seesmic video comments plugin for wordpress works in my default wordpress theme. But when I change the theme to the one I want to use, it shows the only the “Or add a video comment with seesmic” link below the comments box but it is not clickable. Maybe the seesmic js is not loading for this theme. It is a small fix which should take a few minutes and I can pay $10 or max $20 for it. I have a MySql database which has around 32,000 funeral homes in the database which has all the information about the funeral home…name, state, city, etc. I want this database to work with my homepage and what I mean by that is on my homepage I have all the states in the USA listed, I want them to be links and when you click on a state for example “Texas” it would then pull up all the cities in the database that are located in Texas on next page, then if they clicked a city on that page located in “Texas” for example “Houston” it would pull up all the funeral home names that are located in that city. Pretty simple database pull, but I mainly want to use it for SEO. I already have this same thing done on another site of mine but is done in a search box to find the information…I am looking for a more SEO friendly way.

I have started using a template named “project management database” in the MS Access 2007 program. I need to add about 10 more fields when the screen comes up for a new project. Some of those fields will need to have multiple entries.

I would like the video of my choice to be playing on the player, but to also have other videos on the sidebar or on the bottom for visitors to choose what other videos they want to watch. archived. video folders.

i would also like a “photos” section, with some flash integrated, but not too much to take away from the actual photos. photos will also be updated constantly as well, so i will need the authority to upload and update photos when needed. archived, photo folders.

The site for all of this basically functions in two parts – a community part, powered by WordPressMU, and a photo sharing part, powered by Ktools.net’s Photostore.

1) CSS wizardry: I need a css menu designed to replace the header of the existing Photostore header. The current Photostore nav is static, but the community site has a CSS dropdown menu. I need them to match. So, basically a header with a sweet CSS menu and snazzy dropdowns. The options will probably need to change depending on whether the visitor is logged in.

2) WordPress MU template. I need a WP template – actually an adaptation or tweak of an existing template or template framework would be fine. I’m open to suggestions. This will need to feature AdSense ads, and perhaps integrate content from a category of another blog in the site. This will be the default template for all the bloggers on the site. I’m not totally sure what the options are for best operation. Clean, crisp, clever and simple. There will need to be spam protections.

3) CSS Wizardry (2): The Photostore from Ktools can use some cleaning up, but it largely a table-driven site. I’d like some analysis and suggestions about how to make it look like a more modern (Web 2.0-y or whatever). Again, open to suggestions.

This website is selling memberships, you pay to be in the online directory. Who are our paying customers? Mostly this is from age 26 and younger. The members are in the music business and the television business. So those kind of pictures / images should be included.

Visitors come to locate in our database, somebody close to him/her in location. They will search on many or only a few fields. Similar to dating site. They are looking for somebody who will help them with personal assistance.

This Joomla “simplemembership” component is already installed, but maybe needs adjusting or tweaking. Anybody visiting the website can search the database, no need to register or sign anything. Right now, it is asking for you to login and I don’t want that. Can you fix it?

The database will then show who is closest match. It will show all data about the members they were seeking. A large photo, and contact information. You will seek some free Joomla 1.5 templates from template monster (etc), or create your own if you have the software skills (http://www.artisteer.com/).

This is a new Joomla install, so no Articles are currently available. But you will make room for some future growth. There is no guarantee that anybody will want to pay money to be in this database. That means I will not pay a large fee for you to make the site look pretty. BUT!
