Wikipedia Page…. Templates

Need to create a template page for easy inputs into my wikipedia page (already created).
A) Users will need to open my Wikipedia page and enter very basic information into a the fields: Address, City, ST, Name of Gas Station, Price of Fuel for type of fuel.
B) Create a list of select boxes for the user to check if the Gas Station has Car Wash, Air, Service Stations, etc.
C) Upload a few images of maps and pictures of station
Once user is done they need to push a save button and the information they filled out is automatically added to GasPriceWiki.org WikiPedia database page.
