Video Upload And Player

I need a script to upload, convert to flv, and play videos. You will be adding this to an existing site with the following already installed:
1.MySQL
2.PHP 5.2.6
3.FFmpeg
4.FFmpeg-PHP
5.Mplayer + Mencoder
6.flv2tool + GD Library
7.Libogg + Libvorbis
8.LAME MP3 Encoder
I will also need you to modify my upload settings to ensure that I can upload large files.
This is a repost. The original programmer ripped me off. The incomplete project is included for you to finish.
