Alter & Setup A Blog

I need someone who can alter an existing template to create a blog for me. This should be very easy and quick work for the right person. I will provide you with the template, and a screenshot that gives you an idea of what I want. I need someone who will create a visually attractrive optin box ( I will show you what I want). The person who is hired should have no problem setting up the blog and making it easily manageable for me.
