Online Survey – Joomla

We would like an online survey component for Joomla set up or a stand alone survey set up in order to send out customer satisfaction surveys after we are finished dealing with clients.
The points to survey on will be provided. What we need is the online form that captures the answers and it should be emailed to an email address and easily be printed with a pre formatted style.
