Lead Form And Sms Integration

We have an auto classifieds site for which we need lead generating form to be integrated with the classified ad pages, landing pages and as widgets to be placed on the blog and on partner sites. 3-4 databases for different types of leads need to be made along with one main database for used car dealers. SMS integrated is needed to disseminate the leads. Will use API to send SMSes and short code to receive SMSes. Some more minor site modification will have to be done to accomodate lead generation.
Only bid if you are PHP/My SQL experts and have done SMS API integration earlier. Bidders with no review/feedback will not be entertained.
