Book Cover Dvd Cover Package

I am creating my first information product and I need someone that can design the following.
1. Actual Book Cover
2. Actual DVD Cover (including back and side of the case)
3. The DVD itself (the design on the actual dvd)
4. Images of the full package together for promotions
5. A header for the website
6. A mini book image of the book
7. Mini images of the DVD case with the DVD in front of it
Need most of the images in multiple sizes. Must be creative!
