Php Updating Form Page

In need of some php help, creating a php form that mimics the functionality of the form in the link , see attachment. The site owner specifically wants the form process to be step by step instead of long page forms. But instead of having mulitple form pages (done in this case in html) , the form would update based on the user selections. Also, there are hidden variables that would populate into salesforce.com, based on each answer.
