I need to use the format of this site

to make a similar php order form with 4 steps
1/ Choose product options (4 fields)
2/ Update personal info (like step 1 of http://tinyurl.com/lxd25w)
3/ Upload images files (use swfupload or ajax upload code so that we can upload multiple images at once)
4/ Complete payment by paypal
Need admin with simple order manager to view date time and transaction #id, search order by email or #id
Some slight modification after you’ve done is required. This is a fairy simple job, could be done in 3-4 hours by an expert of php. Please give me your demo after making bid. I will select bid quickly..
