Age Verification Code

Hello,
I have a script that is installed on another website of mine I need help installing it onto another site.
It is an age verification script that displays once per session and I need someone to help me work it into the templates throughout the site. Please help. I found a place that it worked but the file became currupt so some minor tweaking or placing in the right place is what I am looking for.
