Data Entry/extract Into Excel

We require someone to produce an xls (MS Excel) file of listings for occupational therapists, physiotherapists, rehabilitation services from the Australian Yellow Pages url http://www.yellowpages.com.au.
Details required from 8 regions to be advised.
Each record contains following fields:
Name,
address,
city,
postcode,
telephone number.
Accuracy of entry in particular spelling is essential. Result can be achieved either by direct entry or scanning but successful bidder must check and guarantee accuracy of finished file records. Allowance of duplicate records to be less than 1%.
