Widget Needed (flash)

I want the visitor of my site to be able to click a link or image that basically says “click here to leave feedback”…
and once they do that we open a new window or ajax popup with flash code that accesses the webcam and computer microphone.
at that point the user should be able to click “record” and start speaking. we record their feedback and keep it somewhere on the server.
ideally, we want to record full video feedback from the site visitor
but if that’s not possible we need AT LEAST a picture of the visitor
and an mp3 recording of the feedback…
