Convert Word Doc To Pdf

Hello,
I have a word document that i need converted to a pdf. The word document has lines in it that say things like: Name, Address, City, etc. I would like to make it so that when i open the pdf file, i can type things into all of these field and then save the pdf file with the typed information or print it.
