Custom Joomla 1.5 Template

Hello.
Our Joomla expert has left us and we need a Joomla 1.5 template based on the attached file. Sorry it is not in html or psd format. All our former employee left us was the attached pdf layout.
We need the template to look as close to the attached layout as possible. What will be important is the placement of the modules.
We will need the all the files necessary to install on a Joomla 1.5 site.
If this project goes well, we will consider arranging a long-term relationship with the winning bidder.
