Template With Website Form

I need a template for a chair cover hiring site.
Example site: http://www.ambiencevenuestyling.com/
Simple and clean, around 3-4 pages.
Colour scheme, red.
I need a form for the website, it will have around 20 questions, and must have captcha verification.
When the form is submitted it must be sent to two emails, one specified by me and the person submitting the form.
Please post mockups of template without form in PMB, bids will not be considered without mockup.
