Two Css Forms Needed

I have a website that is done – I have a simple form that is on the sidebar of each of my .htm pages. It’s a simple form with 3 text fields. Then, I have a page called “Free Quote” where clients can come, fill out their information and submit it to me to get a free web design quote – I need that form developed from scratch using css. DO NOT USE HTML – IT MUST WORK WITH CSS
